Looks like 'dropsy', a fluid build up inside the fish, treating for IP's as puffer punk said and/or maybe an antibiotic. Trying metronidazole if you can get it might be an idea as it works on IP's and is antibiotic?

Sounds like a worming is in order. My fahaka was growing but slowly until I had a heater issue and then almost lost him. After a succesful worming he looks better then ever great colours and growing quickly

Really difficult to make out from the image, if it looks hairy in real life then maybe ich. Although its in a spot the fish could knock? Have any more spots developed? Is there any sign of injury to the skin?

I would agree with the anything and everything quote above. I tend to go for shade tolearnt plants below and floating plants on the suface. I use plants to control nitrates and I scoop out 50% of the floating plants every so often. This way they work like a nitrate sponge.

I have never kept dwarf puffers but other species and read a fair bit on them. They are on my radar for a breeding project. I got live tubiflex from my local fish shop and spread them through the tank, my puffer loves digging for them. Likewise with a couple of corkscrew snail varieties I have bred. Plants, wood, etc to hunt along to make the tank more interesting. I am also not sure the mesh divider will hide the puffer...

I feed mine a mix diet, uncooked whole shrimp (sold in big bags for humans in the local supermarket), bloodworm, white shrimp, tubiflex, worms, apples snails (I breed)

Your not invisible, many of the views would have been people like me who read the post but wouldn't know any more than you. Both fish going down together sounds like water/tank issue to me. You said you have seen this before?, what happened last time?

Looks like an injury to me, possibly some melafix or for now soft food, a few jucy worms are always appreciated

Took my advice from an experienced show Discus breeder. He felt it would be unsafe in food. The dosing for a 450lt tank was only 50mg a higher dose could hurt the liver. Dosed with metro soaked ghost shrip tonight. All looking good.
