That looks like a pimple. I didn't know puffers got zits. Maybe try washing his face twice a day with clean soapy water and tell him not to touch it because the oils will just make it worse.

I would go back to the lfs asap and get some of their established filter media water whatever and the cycle time will be greatly reduced. also feed as little as possible for the first bit because the tank is not yet established enough to take care of a larger sized bio load.

HOw did the dentistry go? I have always wondered how grabbing a fahaka and trimming his teeth would go. I think i would ware chain mail gloves like meat cutters use.

Could you post a picture of this guy id love to see him.

I think he sounds stressed out. New puffers often take a fair amount of time to adjust to their new environment.

I would suggest doing 50% water changes weekly since you said it seems to help.

Overfiltration never hurts anything. but i would recomend a powerhead in that tank and agree with the lr and skimmer idea.
