It seems to be a sad story. I've lost two dps, one oto, one L204 plec and my blue Betta. Getting bonkers now.
I've been through the esha Flukeworm and tapeworm cycle and now on to esha ndx anti nematode.
There's no sign of anything wrong when they die. :/

My l204 is nocturnal and is a bit flappy with it's fins. No problems though.
The ancistrus can be impossible to spot against wood or gravel.
