If the water has any kinds of pollutants or pesticides, they might end up in the feeders. Even if they aren't affecting the feeders, they will build up in the puffer over time. This is, of course, a non-issue if you know the river is clean.

Doesn't specifically mention pufferfish, but this article says that basically any fish can get dropsy: http://freshaquarium.about.com/od/termsandtables/p/Dropsy.htm It does look just like amphibian bloat, which I think is identical to dropsy. But neither this nor that article are necessarily meaning...
