It's dying from bad water conditions probley seen as you don't have a clue.

Have you tested your water? How big is the tank and how many puffers?

NH4=0, NO2=0, NO3=50mg/l

Should be be below 20. Do a 70% water change.

Is that about 70 gallons? Does it have a real filter with the 14 puffers? You should have way less puffers in there. 5 gallons is not enough for anything but mabie one dwarf puffer.

This is why you need to listen to us tk. If you don't start improving their living conditions they will keep dying

You need to start rehoming most of those puffers, if you don't have any suitable spare tanks. Are they all GSP's? Because they need marine conditions to thrive.

Your puffers sick because there are too many, in too small a tank and not the right environment for that type of puffer. If no changes are made, you will keep having sick or dying puffers :(

It could be nitrite poisoning or Ich.
