i know its hard to take a picture of a fish, but does your camera or phone have a "burst shot" mode? where it will take a few quick pictures of a moving object? if not hopefully you can try to get a more in focus shot so someone can help. or try google images for "gill parasite" ...

pretty much what everyone has said so far +1. a good way to provide hiding spots would be pvc piping or clear tubing. i have seen on youtube a really cool setup where someone put the tubes down in the sand making a whole passageway for the eel, was pretty awesome!
